Assassination warning in March?

In another Forum, a member was asking about an assassination warning the Secret Service received in March of 1963.

Based on what is in the HSCA Report, the details might be found in footnote number 17 of Chapter XI of Volume 10 of the HSCA Hearings.

If you scroll down to the bottom of the page, footnote number 17 refers to a Report filed by the Chicago Secret Service field office.

Is this the Report where the Washington office ordered the Chicago office to send them all copies of the Report and any field notes and never discuss it with anyone else?

Has anyone here ever seen this Chicago Secret Service Report?
