Receipt for Rifle

In reading David Joseph's fine article on the rifle at  Kennedys and King, I note that he produces a part of the USPS manual which says that when a firearms package is sent, the person it is sent to has to sign a receipt which is called a form 2165.

I have been looking for that form for a long time.  Can anyone produce it?

Also, has anyone ever seen the part of Oswald's  PO box application where he was to mark down if another person besides himself could get mail there?

David says they do produce it in the volumes, but I cannot find it.

I know the FBI did an inquiry on this and they said he did not designate anyone.

Did that part of the form ever surface anywhere or did Harry the xxxx Holmes just deep six it?

No, the application still exists.  But the part where you assign someone else permission to enter your box was supposed to be missing.

 

But in his article, David alludes that maybe the Commission screwed up and published it.
